Who is this section for?

This section in the Documentation is the Designer Manuals. These manuals are aimed at anybody who is designing themes for PyroCMS either to build sites with or to sell on the Add-on Store.

How difficult is it?

If you have a good understanding of HTML and CSS then you should be able to create themes for PyroCMS. Creating a PyroCMS theme should be no harder than designing a regular HTML template. In fact you can easily convert an existing template so that PyroCMS can install it. Themes make extensive use of Tags so no PHP knowledge is required for designers.
